  Service List

  All of the following services as are included in your hospice care.  You do not have to pick and choose
  services...it is all provided as part of the care plan.  At RH, we strive to make you as happy and
  comfortable as we can.  Part of that mission includes making every attempt to keep you with the
  same team of caregivers.  Change is stressful, and once you have become familiar with certain
  team members, knowing they will visit you regularly can be a comfort.  We also offer a free,
  INTERVENTIONS
program for those who are not yet hospice appropriate but need support. 

 

    Nursing Care  Pharmacy

  RNs and LPNs will provide you with hands-on
  care to help ensure you are
comfortable and   
  functioning at the highest level.  Nurses discuss  
  medication changes, special equipment, and
  other concerns with
your physician and our 
  medical director.  Nurses are on call
24-7 in
  case of emergency or pain crisis.  Nursing
  visits occur 2 to 3 times a week.

 

 

Hospice takes care of providing and paying for your medications and instruct you or a caregiver on their proper use.  RH has an in- house pharmacy, or we will use your preferred pharmacy of choice. 

 

 

   Homemaker Services

 Chaplains

  Homemaker services available include:

  Vacuuming, dusting, cleaning bathrooms and  
  kitchens, light meal preparation, bathing,
  personal care, and dressing.  Homemaker
  services are generally provided 2 to 3 days
  a week.

 

 

Our chaplains are well versed in multiple faiths, and denominations.  They are available to you and your family for spiritual counseling and guidance through this difficult time. Chaplains visit at least one time per month, more frequently as requested.

 

 




 Medical Social Workers
 Community Service
 Coordination

  Medical Social Workers can help you to
  identify and apply for pay sources such as
  Medicaid and Medicare, help with financial  
  concerns, and provide emotional support to the 
  patient and family. They can help write letters
  to family members and provide as commun-
  icators with family members. MSWs visit at
  least one time per month, more frequently as  
  requested.

 

 

Regional Care Hospice can arrange for a variety of community services to help you.  Meals on Wheels, transportation to a doctor, respite services, and others can be scheduled to provide support for you and your caregivers.  Services that are necessary can be discussed during our initial visit, and can be added to or changed at any time.

 

 

     Bereavement Program    Volunteers

  An important part of hospice is a good
  bereavement program.  RH's bereavement
  program continues for 13 months after a patient  
  passes.  Phone calls, visits, letters, and  
  informational mailings help family members
  cope with the passing of a loved one.  Special 
  events are not forgotten: birthdays,
  anniversaries, and other days special to your
  family will be commemorated with them and the
  feelings that may be present at such occasions
  are highlighted.  Grief is a natural process, and
  each person handles it differently.  Our
  counselors, volunteers, and social workers are
  available to family at any time.

 

Hospice volunteers are special people who are trained to provide you and your family with additional support.  Volunteers can provide relief for family members by sitting with a patient so that a family member may have some time off.  Volunteers will also spend time just being a friend.  Playing cards, reading to you, or just a shoulder to cry on, volunteers are the life blood of a hospice program.  Volunteers are available to you for support and a little bit of fun!. 


  INTERVENTIONS 
This program is available for persons who:

· are not quite ready for hospice but will need services soon;

· Have received services from RH but stopped receiving services for various reasons,
such as health improvement or surgery

· If the process of qualifying for services is lengthier than the expected time frame before
the patient passes away

  Interventions includes visits form MSWs and Chaplains, as well as nurse consultation visits. 
  Nurses do
not provide hands-on care in this program, but they can make recommendations to
  your doctor, the facility doctor, or our medical director.  Support continues for patient and family
  on an as needed basis.  Interventions is
